1. Application Letter addressed to the University President, Dr. Nancy T. Pascual, and submit to the University Human Resource Management Unit, URS Morong Campus, Morong, Rizal, not later than October 6, 2023;

2. Fully accomplished under oath Personal Data Sheet (CS Form 212, Revised 2017) with recent passport-sized photo which can be downloaded at www.csc.gov.ph;

3. Work Experience Sheet which can be downloaded at www.csc.gov.ph;

4. Photocopy of Transcript of Records, including Diploma, Special Order from

undergraduate degree to highest education attained as declared in the PDS;

5. Photocopy of Certificate of Employment/Service Contract as proof of experience;

6. Photocopy of Certificate of Relevant Tertiary Trainings/Seminars conducted by Professional Organization;

7. Photocopy of Certificate of Relevant Eligibility/Rating or Professional License (if applicable); and,

8. Proof of Published Patented Materials relative to Research, Development, Extension and Production Engagement (if any).
